Surgical Procedure

Chief Complaint

  • Patient presented with missing lower left and right molars and requested implant-supported replacement for functional rehabilitation.

Clinical Findings

  • Missing teeth at #37, #46, and #47

  • Adequate bone volume for implant placement

Buccal bone deficiency anticipated at #46 and #47, with possible need for bone grafting

Treatment Plan

  • Implant placement at sites #37, #46, and #47

  • Buccal bone grafting at #46 and #47 as required

  • Healing abutment placement for transmucosal healing

  • Post-operative CBCT to confirm implant position and nerve safety

Pre-operative Assessment

  • CBCT imaging reviewed and implant positions planned

  • Risks, procedures, and potential need for bone grafting explained

  • Patient demonstrated understanding and was agreeable to proceed

Informed consent obtained for implant placement at sites #37, #46, and #47

Anesthesia

Local anaesthesia administered at all surgical sites

Surgical Procedure

Implant Placement at #46 and #47

  • Patient cleaned and draped under sterile conditions

  • Buccal incision made and full-thickness flap raised

  • Implant osteotomies prepared using implant drills and osteotomes

  • Implants placed at #46 and #47 with appropriate primary stability

  • Circumferential bone profiling performed

  • Buccal bone grafting carried out using collagen-based artificial bone graft material

  • Healing abutments placed

  • Flap released and primary closure achieved with sutures

  • Hemostasis confirmed and post-operative instructions given

Implant Placement at #37

  • Local anaesthesia administered

  • Buccal incision made and flap raised

  • Implant osteotomy prepared using implant drills and osteotomes

  • Implant placed at #37 with satisfactory primary stability

  • Circumferential bone profiling performed

  • Healing abutment placed

  • Flap released and primary closure achieved with sutures

  • Hemostasis confirmed and post-operative instructions given

Post-operative Assessment

  • Post-operative CBCT taken

  • Implant alignment and positioning confirmed to be satisfactory

  • Inferior Dental Nerves not involved in implant placement

  • Patient reassured and satisfied with the procedure outcome
