Patient presents for implant-supported replacement of missing lower anterior teeth (#41, #42, #31, #32).

Clinical Findings

  • Missing #41, #42, #31, #32

  • Full-arch digital scans completed (upper & lower)

  • Planned PFM Ti alloy bridge over implant sites

  • CBCT reviewed; adequate bone volume for placement

  • Risks, benefits, and procedures explained; patient consented

Treatment Plan

  • Placement of four implants at #41, #42, #31, #32

  • Bone grafting to support ideal implant stability

  • Temporary jointed bridge for immediate restoration

  • Post-op CBCT to confirm implant angulation and graft integration

Pre-operative Assessment

  • CBCT reviewed for bone density, anatomy, and surgical planning

  • Implant positions mapped

  • Patient understood the procedure, risks, and alternatives

  • Informed consent obtained

Anesthesia

  • Local anesthesia administered

  • Patient cleaned and draped in sterile field

Surgical Procedure

  • Incision made; buccal flap raised

  • Osteotomy sites prepared with implant drills and osteotomes

  • Four implants placed: 3.0mm × 10.0mm Dentium Slim Line Implants at #41, #42, #31, #32

  • Bone grafting performed using Dentium Osteon Collagen II (6.0mm × 10.0mm)

  • Flap released; BSS ×12 placed for primary closure

  • Hemostasis achieved; POI given

Temporalization
  • Temporary 4-unit jointed bridge fabricated

  • Cemented with temporary bond

Post-operative Assessment

  • Post-op CBCT taken

  • Implants well-aligned; optimal positioning

  • Bone graft dense and well-packed

  • Inferior dental nerve not involved

  • Patient satisfied with stability and appearance of temporary bridge
