Chief Complaint
Patient requires replacement of missing teeth #14, #26, #46, and #47 with implants.
Clinical Findings
Missing teeth #14, #26, #46, and #47.
Pre-operative CBCT reviewed.
Bone depth and structure assessed, with maxillary sinus evaluated for #26.
Treatment Plan
Placement of implants at sites #14, #26, #46, and #47.
Bone grafting with a combination of artificial and autogenous bone material.
Pre-operative Assessment
Risks and procedures explained to the patient.
Replacement alternatives discussed; patient opted to proceed with implant placement.
Informed consent obtained.
Anesthesia
Local anesthesia administered.
Surgical Procedure
1. Patient cleaned and draped.
2. Incision made and buccal flap raised, extended to the ascending ramus in the 4th quadrant.
3. Implant sites prepared using implant drills and ridge-expanding drills.
4. Bone harvested from all osteotomy sites and the ascending ramus; stored in sterile saline.
5. Implants placed:
- #14: Dentium Superline I 4.5mm x 8.0mm with 4.5mm (S) healing abutment.
- #26: Dentium Superline I 4.0mm x 8.0mm with 4.5mm (M) healing abutment. Maxillary sinus lifted internally to ensure adequate bone depth.
- #46: Dentium Superline I 4.0mm x 8.0mm with 4.5mm (S) healing abutment.
- #47: Dentium Superline I 4.0mm x 8.0mm with 4.5mm (S) healing abutment.
6. Bone grafting applied to buccal sites of #14, #46, and #47 using EthOss 1.0cc artificial bone graft mixed with harvested autogenous bone.
7. Flap released and BSS applied for primary closure:
- #14: BSS x 05
- #26: BSS x 03
- #46 and #47: BSS x 06
8. Haemostasis achieved and post-operative instructions provided.
