Surgical Procedure

Chief Complaint

Missing upper left premolar and molar (#25 & #26) requiring implant-supported replacement.

Clinical Findings

  • Edentulous spaces at #25 and #26

  • Adequate ridge volume with need for ridge expansion

  • Sinus proximity requiring internal sinus lift

  • Pre-operative CBCT confirmed suitable anatomy for implant placement with augmentation

Treatment Plan

  • Implant placement at #25 and #26

  • Ridge expansion and internal sinus lift

  • Sinus augmentation with autogenous and collagen-based bone graft

  • Buccal bone grafting

  • Healing abutment placement

  • Post-operative CBCT evaluation

Pre-operative Assessment

  • CBCT reviewed; implant positions planned digitally

  • Risks and procedures explained

  • Quotation given

  • Patient fully understood and consented

  • Informed consent signed for implant placement at sites #25 and #26

Anesthesia

Local anesthesia.

Surgical Procedure

  • Patient cleaned and draped

  • Incision made; buccal flap raised

  • Osteotomy preparation using implant drills and osteotomes

  • Ridge expansion and internal sinus lift performed at both sites

  • Sinus augmentation using:
    • Autogenous bone from osteotomy sites
    • Dentium Osteon II collagen artificial bone graft (6.0 mm × 10.0 mm)

  • Megagen AnyOne Implants placed:
    • #25: 4.5 × 8.5 mm
    • #26: 4.5 × 8.5 mm

  • Dentium healing abutments placed:
    • #25 → 6.5 mm (S)
    • #26 → 6.5 mm (S)

  • Additional buccal bone grafting applied over both sites

  • BSS × 5 placed for primary closure

  • Hemostasis achieved; post-operative instructions provided

Post-operative Assessment

  • Post-op CBCT taken and reviewed

  • Maxillary sinus intact post sinus lift

  • Dense bone graft observed

  • Implant angulation and positioning ideal

  • Patient shown postoperative images and satisfied with results
