Patient presented with a missing tooth at position #45.

  • Explained risks and procedures.

  • Patient understood and chose to replace missing #45 with an implant.

  • Pre-operative CBCT taken.

Treatment Plan

  • Implant placement surgery at the missing tooth position #45.

  • Potential bone grafting if necessary.

Pre-operative Assessment

  • Pre-operative CBCT taken.

  • Patient agreeable to proceed with implant placement surgery today.

  • Informed consent obtained.

Anesthesia

Local anesthesia administered: Right Inferior Dental Nerve (IDN), Lingual Nerve (LN), and Long Buccal Nerve (LBN) blocks.

Surgical Procedure

  1. Patient cleaned and draped.

  2. Incision made.

  3. Buccal flap raised.

  4. Implant site prepared.

  5. Placed 3.6mm x 8.0mm Dentium SuperLine III implant.

  6. Healing abutment 5.5mm (Medium) placed.

  7. Bone grafting performed on buccal side of #45 implant using Ethoss 1.0cc artificial bone grafting material.

  8. Flap released.

  9. BSS x 05 placed for primary closure.

  10. Achieved haemostasis and provided post-operative instructions (POI).

Post-operative Assessment

  • Post-operative CBCT taken.

  • Implant position verified as good.

  • Bone graft appears dense.

  • Healing abutment not fully seated.

  • Healing abutment torqued further into implant at 15Nm.
