Chief Complaint
Replacement of missing teeth #46 and #47 with implants.
Clinical Findings
Pre-operative CBCT reviewed.
Implant positions planned and IDN (Inferior Dental Nerve) plotted.
Treatment Plan
Placement of implants for teeth #46 and #47.
Bone grafting using Osteon Collagen II and autogenous bone.
Pre-operative Assessment
Risks and procedures explained to the patient.
Implant positions and IDN plotted.
Patient understood and agreed to proceed.
Informed consent obtained for implant placement at #46 and #47.
Anesthesia
Local anesthesia administered.
Surgical Procedure
1. Patient cleaned and draped.
2. Incision made and buccal flap raised.
3. Implant sites prepared with drills and osteotomes.
4. Bone from osteotomy sites conserved and stored in sterile saline.
5. Implants placed:
- #46: Dentium Superline III 4.5mm x 8.0mm
- #47: Dentium Superline III 4.5mm x 8.0mm
6. Bone grafting performed:
- Autogenous bone harvested and placed on buccal side of #46 implant.
- Osteon Collagen II (6.0mm x 10.0mm) applied over autogenous bone.
7. Flap released and closed with bone grafting material (BSS x 10).
8. Haemostasis achieved and post-operative instructions provided.
