Chief Complaint
Missing tooth at position #45 and a dislodged crown at #17.
Clinical Findings
Tooth #45 missing.
#17 crown dislodged.
#46 full metal crown showed leakage and recurrent caries, with mesial tilting into the space of missing #45.
Teeth #17 and #46 were asymptomatic, not tender to percussion or palpation (NTTP, NTTPp), mobility present, probing depth within normal limits (PD WNL), both had prior root fillings, no abnormal radiolucency at apices.
Treatment Plan
Implant placement at the site of missing tooth #45.
Crown preparation for teeth #17 and #46.
Pre-operative Assessment
CBCT taken to plan implant position.
Inferior Dental Nerve (IDN) plotted.
Risks and procedures explained to the patient.
Replacement alternatives discussed; patient opted for implant placement at site #45.
Informed consent obtained for local anesthesia (LA) implant placement.
Anesthesia
Local anesthesia administered.
Surgical Procedure
Crown Preparation:
#46 full metal crown sectioned and removed.
Crown prep performed on teeth #17 and #46.
Caries at #47 (mesial) treated; composite restoration with shade A3 applied after etching, priming, and bonding.
Impressions taken using Impregum for upper and lower half arch; bite registration with Futar D.
Temporary crowns placed on #17 and #46.
2. Implant Placement Surgery:
Area cleaned and draped.
Buccal flap raised, implant sites prepared with implant drills.
Dentium Superline III Implant (4.0mm x 10.0mm) placed at the site of missing tooth #45.
BSS x 05 sutures placed for primary closure.
Hemostasis achieved, post-operative instructions given.
