Treatment Plan

  • Implant placement at #45 and #46 sites

  • Bone graft augmentation

  • Immediate temporisation with temporary jointed bridge

Pre-operative Assessment

CBCT reviewed prior to surgery.
Implant angulation, positioning, and proximity to adjacent anatomical structures assessed.

Risks including infection, implant failure, swelling, bleeding, paraesthesia, graft failure, and post-operative discomfort discussed.
Px understood and agreed to proceed.
Informed consent obtained for implant placement at #45 and #46 sites.

Anesthesia

Local anesthesia administered to surgical sites.

Surgical Procedure

Px cleaned and draped in sterile manner.

Incision made and buccal flap raised.
Implant osteotomy sites prepared using implant drills and osteotomes.

Implants prepped with Megagen Plasma treatment prior to placement.

  • 4.5mm x 8.5mm Megagen Anyone Implant placed at #46 site

  • 5.0mm x 7.0mm Megagen Anyone Implant placed at #45 site

Temporary abutments connected for both implants.
Abutments temporised with temporary jointed bridge fabricated using composite resin.

Bone grafting performed using Dentium Osteon Collagen II artificial bone grafting material.

Flap released for tension-free closure.
BSS x4 placed for primary closure.
Haemostasis achieved.
POI given.

Post-operative Assessment

Post-operative CBCT taken.
IDN not involved during surgery.
Implant positioning and alignment satisfactory.
Bone graft adaptation dense and stable.
Px tolerated procedure well and left clinic in stable condition.
