Surgical Procedure

Chief Complaint
Patient presented to restore missing posterior teeth on the lower left side for improved function and chewing efficiency.
Clinical Findings
#35 and #36 missing teeth sites indicated for implant-supported restoration.
Adequate bone height and width confirmed via CBCT.
Adjacent teeth healthy and stable.
Occlusion stable.
Treatment Plan
Placement of two dental implants at #36 and #37 to restore function and support future prosthetic crowns.
Pre-operative Assessment
CBCT reviewed to evaluate bone volume and anatomical structures.
Implant positions and angulation digitally planned.
Risks, benefits, alternatives, and post-operative expectations discussed.
Patient expressed understanding and consented to proceed.
Anesthesia
Local anesthesia administered. Patient cleaned and draped for sterile surgical protocol.
Surgical Procedure
Incision made and buccal flap raised for access.
Implant osteotomies prepared with sequential drilling and osteotomes.
Dentium Superline implants placed:
#36: 4.5 × 8.0 mm with 7.5 mm (M) healing abutment.
#37: 4.5 × 8.0 mm with 9.5 mm (M) healing abutment.
Flap released for tension-free closure.
Closed with 4/0 BSS sutures (x3).
Hemostasis achieved.
Post-operative CBCT taken confirming correct implant alignment and seating.

Post-operative Assessment
Patient stable and comfortable.
Good primary closure noted.
Healing abutments in place.
Post-operative instructions given.
Patient satisfied and will return for review.
