Chief Complaint

Patient satisfied with current #36 implant crown; presented for implant surgery at missing #24 and #16 sites.

Clinical Findings

  • #36 implant crown asymptomatic, functioning well; removed as one unit for lab finishing

  • Missing #24 and #16 requiring implant-supported replacement

  • CBCT confirmed adequate ridge height with need for internal sinus lift at #16

  • No pathology noted at proposed implant sites

Treatment Plan

  • Implant placement at #24 and #16

  • Internal (crestal) sinus lift for #16

  • Healing abutments placed at both sites

  • Bone grafting avoided based on intra-operative assessment

  • Post-operative CBCT verification

  • CBCT reviewed

  • Implant positions planned digitally

  • Risks and procedures explained, including sinus lift considerations

  • Quotation provided for implants, prosthetics, sinus lift, and augmentation

  • Patient understood and agreed

  • Informed consent obtained for implant placement

Anesthesia

Local anesthesia.

Surgical Procedure

  • Patient cleaned and draped

  • Incision made; buccal flap raised

  • Osteotomy sites prepared with implant drills and osteotomes

  • Internal sinus lift performed at #16

Implant Placement:

#24:

  • Dentium Superline III 4.5 × 8.0 mm implant placed

  • 5.5 mm (M) healing abutment attached

#16:

  • Dentium Superline III 5.0 × 8.0 mm implant placed

  • 8.5 mm (M) healing abutment attached

  • Bone grafting intentionally avoided due to sufficient bone support

  • BSS × 5 placed at each site for primary closure

Hemostasis achieved; POI given

Post-operative Assessment

  • Post-op CBCT confirmed ideal implant angulation and positioning

  • Sinus lift successful with intact membrane and no perforation

  • Patient satisfied with surgical outcome
