Chief Complaint
Patient complains of pain in the third quadrant and requests extraction.
Clinical Findings
#37 shows buccal swelling with deep buccal and lingual probing depths.
#38 is impacted with a severe food trap between #38 and #37.
CBCT taken reveals:
#37 has periodontal-endodontic involvement.
#38 is impacted.
#28 is non-functional.
Treatment Plan
Advised extraction of #37, considering extraction of #38 and #28.
Pre-operative Assessment
Patient chose to proceed with extraction of #37, #38, and #28.
Risks and procedures were explained, and the patient understood and agreed.
KIV operation for #38 estimated
Informed consent was obtained.
Anesthesia
Local anesthesia was administered successfully for the extraction of #28 and #37.
Surgical Procedure
Extraction of #38 was complicated with retained mesial and distal roots, unable to be fully extracted.
Additional CBCT was taken, and the inferior dental nerve (IDN) was traced.
Local anesthesia was administered again for the removal of the retained roots of #38.
Patient was cleaned and draped.
Incision made, buccal flap raised.
Bone guttering performed, and #38 mesial and distal roots were removed.
Currettage and removal of granulation tissues.
Irrigation with saline.
BSS x 05 placed for primary closure.
Haemostasis achieved and post-operative instructions given.
