Impacted Wisdom Tooth Removal

Surgical Removal

Chief Complaint

Patient complains of pain and swelling distal of tooth #37.

Food trap distal of #37 causing infection, discomfort, and swelling.

Possible impacted tooth #38 noted on probing distal of #37.

Clinical Findings

Pre-operative CBCT shows fully buried and impacted tooth #38.

Tooth #38 is close to the inferior dental nerve (IDN).

Treatment Plan

Advised removal of impacted tooth #38 under local anesthesia (LA).

Patient agreed to the procedure.


Pre-operative Assessment

Risks of parasthesia to lower lip and tongue explained.

Procedures discussed, and patient provided informed consent.

Anesthesia

LA block to left IDN, LN, and LBNAdditional LA infiltration around tooth #38.

Surgical Procedure

  1. Patient cleaned and draped.

  2. Incision made and buccal flap raised.

  3. Bone guttering performed.

  4. Tooth #38 sectioned and elevated in 5 pieces.

  5. Curettage and saline irrigation completed.

  6. BSS x 05 placed for primary closure.

  7. Haemostasis achieved.

Post-operative Assessment

Post-operative CBCT confirms no tooth remnants.

IDN intact and not involved in surgery.

Patient reassured.
