Dental Implant Placement with Retained Root Removal and Immediate Temporary Crown for Missing Lower Right Lateral Incisor (#42)

Surgical Procedure

Chief Complaint

Patient presented for replacement of a missing lower right lateral incisor (#42) with a dental implant.

Clinical Findings

  • Missing tooth #42.

  • Pre-operative CBCT reviewed for implant planning.

  • Retained root identified at the #42 site, requiring removal prior to implant placement.

  • Implant position and angulation planned before surgery.

Treatment Plan

  • Removal of the retained root at site #42.

  • CBCT-guided dental implant placement.

  • Buccal contour augmentation using autogenous bone harvested during implant site preparation.

  • Placement of an implant abutment.

  • Immediate temporary crown.

  • Post-operative CBCT to verify implant positioning.

Pre-operative Assessment

Pre-operative CBCT was reviewed to assess the implant site and surrounding anatomical structures.

The patient was informed of:

  • The need to remove the retained root before implant placement.

  • The implant placement procedure.

  • Surgical risks, expected healing, and possible complications.

  • Benefits and limitations of immediate temporisation.

The patient understood the treatment plan and provided informed consent.

Anesthesia

Local anaesthesia administered to the surgical site.

Surgical Procedure

The patient was cleaned and draped under sterile protocol.

  • Retained root at site #42 was removed.

  • Crestal incision made and buccal flap elevated.

  • Implant osteotomy prepared using sequential implant drills and osteotomes.

  • A Megagen AnyOne Mini implant (3.0 mm × 10.0 mm) was placed with satisfactory primary stability.

  • EZ Post abutment connected and secured to 15 Ncm.

  • Autogenous bone harvested during implant site preparation was grafted onto the buccal aspect of the implant to support ridge contour.

  • Flap released and primary closure achieved with seven interrupted sutures.

  • Haemostasis confirmed.

Immediate Temporisation
  • A temporary crown was fabricated and cemented using temporary cement.

A post-operative CBCT confirmed:

  • Accurate implant position and angulation.

  • Inferior Dental Nerve remained uninvolved.

  • Satisfactory implant alignment.

Post-operative Assessment

  • Implant demonstrated good primary stability.

  • Temporary crown remained stable.

  • Post-operative CBCT confirmed ideal implant positioning.

  • Inferior Dental Nerve remained intact.

  • Patient was satisfied with the surgical outcome.
