Dental Implant Placement with Bone Grafting for Missing Lower Right First Molar (#46)
Surgical Procedure

Chief Complaint
Patient presented for replacement of a missing lower right first molar (#46) with a dental implant.
Clinical Findings
Missing tooth #46.
Pre-operative CBCT reviewed to evaluate bone volume and identify the course of the Inferior Dental Nerve (IDN).
Implant size, angulation, and restorative position were planned prior to surgery.
Treatment Plan
CBCT-guided dental implant placement at site #46.
Placement of a healing abutment.
Bone grafting around the implant using collagen-containing synthetic bone graft material.
Post-operative CBCT to verify implant positioning and relationship to the Inferior Dental Nerve.
Pre-operative Assessment
Pre-operative CBCT was reviewed to assess available bone and accurately trace the Inferior Dental Nerve.
The patient was informed of:
The implant placement procedure.
Surgical risks and possible complications.
Expected healing process and subsequent restorative stages.
The patient understood the treatment plan and provided informed consent.
Anesthesia
Local anaesthesia administered to the surgical site.
Surgical Procedure
The patient was cleaned and draped under sterile protocol.
Crestal incision made and buccal flap elevated.
Implant osteotomy prepared using sequential implant drills and osteotomes.
A Dentium Superline III implant (7.0 mm × 10.0 mm) was placed at site #46 with satisfactory primary stability.
A healing abutment was connected.
Bone grafting was performed around the implant using Dentium Osteon II Collagen synthetic bone graft material to support the surrounding bone contours.
Flap released and primary closure achieved with four interrupted sutures.
Haemostasis confirmed.
A post-operative CBCT confirmed:
Accurate implant position and angulation.
Inferior Dental Nerve remained uninvolved.
Satisfactory implant alignment.

Post-operative Assessment
Implant demonstrated good primary stability.
Healing abutment remained stable.
Post-operative CBCT confirmed ideal implant placement.
Inferior Dental Nerve remained intact.
Patient was satisfied with the surgical outcome.
