Buried and Impacted #48 Removal Surgery

Surgical Removal

Chief Complaint

Patient presented with pain and swelling on the lower right side. Requested removal of tooth #48.

Clinical Findings

  • Tooth #48 was buried and impacted.

  • Localized swelling noted around the area.

  • No trismus or lymphadenopathy.

  • Pre-operative CBCT confirmed impaction position.

Treatment Plan

  • Local anesthesia (LA) operative removal of #48.

  • Risks, benefits, and potential complications (including paraesthesia to lower lip and tongue) explained.

  • Patient and father understood and agreed to proceed.

  • Informed consent obtained.

Pre-operative Assessment

  • CBCT reviewed for root morphology and proximity to inferior alveolar canal.

  • No active infection contraindicating surgery.

  • Medical history unremarkable.

Anesthesia

LA block administered: Right IDN, LN, and LBN.

Surgical Procedure

  • Patient cleaned and draped under aseptic protocol.

  • Incision made and buccal flap raised.

  • Bone guttering performed to expose tooth.

  • Tooth sectioned and removed in 6 pieces.

  • Curettage and saline irrigation performed.

  • Primary closure achieved with 4/0 BSS × 6 sutures.

  • Hemostasis achieved.

  • Post-operative instructions given.

  • Intra-operative photos taken.

Post-operative Assessment

  • Post-op CBCT taken — no tooth remnants observed.

  • Inferior dental nerve (IDN) intact, not involved in surgery.

  • Swelling and bleeding controlled.

  • Patient reassured and reviewed post-operatively.
