Chief Complaint

Patient reported pain and swelling in the lower left posterior area.

Clinical Findings

  • Tooth #38:

    • Vertically impacted.

    • Inflamed operculum due to traumatic bite from tooth #27.

  • Tooth #28:

    • Mesially impacted.

Treatment Plan

Wisdom Tooth Surgery for removal of vertically impacted #38 and mesially impacted #28 under local anesthesia (LA).

Pre-operative Assessment

  • Pre-operative CBCT taken.

  • Risks of paresthesia to lower lip and tongue, oro-antral perforation (OAP), and oro-antral fistula (OAF) explained to the patient.

  • Procedures thoroughly explained.

  • Patient understood and agreed to proceed.

  • Informed consent obtained.

Anesthesia

  • Left IDN, LN, and LBN blocks administered.

  • LA infiltration at #28.

Surgical Procedure

  1. Preparation:

  • Patient cleaned and draped.

2. Removal of #38:

  • Incision made and buccal flap raised.

  • Bone guttering performed.

  • Tooth sectioned and elevated out in two pieces.

  • Curettage and irrigation with saline.

  • BSS placed for primary closure.

  • Haemostasis achieved.

  • POI given.

3. Removal of #28:

  • Incision made and buccal flap raised.

  • Bone guttering performed.

  • Tooth sectioned and elevated out in two pieces.

  • Curettage and irrigation with saline.

  • BSS placed for primary closure.

  • Haemostasis achieved.

  • POI given.
