Chief Complaint
Patient complains of pain in the 4th quadrant.
Clinical Findings
#47 has deep caries extending to the pulp.
#47 is very tender to palpation and percussion.
Significant mobility and deep probing depth noted on the distal aspect.
Horizontal impaction of #48 observed.
#47 shows bone loss extending to the apices due to impaction of #48.
Treatment Plan
LA XAP #47 due to sub-crestal buccal caries and pulpitis affecting #47.
LAOP #48 due to distal impaction affecting #47 with periapical pathology.
Pre-operative Assessment
PA and CBCT imaging performed to assess the condition and position of #47 and #48.
Risks and procedures for both LA XAP #47 and LAOP #48 explained to the patient.
Patient agreed to proceed with treatment today after understanding the plan.
Anesthesia
LA block administered to the right inferior dental nerve (IDN), lingual nerve (LN), and buccal nerve (LBN) for adequate anesthesia.
Surgical Procedure
Patient prepared, cleaned, and draped for surgery.
Incision made, buccal flap raised, and bone guttering performed.
Tooth sectioning executed for both #47 and #48.
#47 removed in fragments due to extensive caries.
#48 sectioned and elevated in three pieces.
Saline irrigation, curettage performed, and BSS x 07 placed for primary closure.
Hemostasis ensured and post-operative instructions provided.
